Introducción y objetivos

El electrocardiograma (ECG) es la prueba diagnóstica más utilizada en las consultas de cardiología. El temblor de miembros superiores constituye una causa frecuente de artefactos que puede comprometer su calidad e interpretabilidad, conduciendo en ocasiones a errores diagnósticos de consecuencias clínicas graves. El objetivo de este estudio fue evaluar si la maniobra puño cerrado a la espalda (MPCE), una técnica de nueva descripción mejora la calidad del registro electrocardiográfico en estos pacientes.

Métodos

Se incluyeron consecutivamente 117 pacientes con temblor importante de miembros superiores durante la realización del ECG. A cada paciente se le realizaron dos ECG: uno con la técnica estándar (basal) y otro con la MPCE. La calidad se evaluó con una escala ordinal de cuatro niveles por dos cardiólogos independientes, con desempate por un tercer cardiólogo.

Resultados

La MPCE mejoró significativamente la calidad del ECG respecto a la técnica basal (Wilcoxon W=257,0, McNemar exacto: p <0,0001 en ambos). La proporción de ECG de calidad aceptable aumentó del 66,4% al 87,1%. El 74,4% (IC95%, 58,9-85,4) de los ECG basales de mala calidad se rescató a calidad aceptable.

Conclusiones

La MPCE es una maniobra sencilla que mejora significativamente la calidad del ECG en pacientes con temblor de miembros superiores, rescatando 3 de cada 4 registros basales de mala calidad. Su implementación rutinaria puede reducir la tasa de ECG no interpretables y mejorar la seguridad diagnóstica.

Abstract
Introduction and objectives

The electrocardiogram (ECG) is the most widely used diagnostic test in cardiology outpatient clinics. Limb tremor — both resting and intentional — is a frequent source of artifacts that may compromise ECG quality and interpretability, occasionally leading to serious diagnostic errors. This study aimed to evaluate whether the closed fist behind the back maneuver (CFBM), a newly described technique, improves ECG recording quality in these patients.

Methods

A total of 117 consecutive patients with significant upper limb tremor during ECG recording were enrolled. Each patient underwent 2 ECG: 1 with the standard technique (baseline) and 1 with the CFBM. Quality was assessed using a 4-level ordinal scale by 2 independent cardiologists, with disagreements resolved by a third cardiologist.

Results

The CFBM significantly improved ECG quality compared to the baseline technique (Wilcoxon W=257.0; exact McNemar: P <.0001 for both). The proportion of acceptable-quality ECG increased from 66.4% to 87.1%. A total of 74.4% (IC95%, 58.9–85.4) of poor-quality baseline ECG were rescued to acceptable quality.

Conclusions

The CFBM is a simple maneuver that significantly improves ECG quality in patients with upper limb tremor, rescuing three out of four poor-quality baseline recordings. Its routine implementation may reduce non-interpretable ECG rates and improve diagnostic safety.
