To explore the association between left atrial reservoir strain (LASr) and postoperative atrial fibrillation (POAF) in adult patients undergoing cardiac surgery POAF is a common complication of cardiac surgery in adults. Although it is associated with adverse outcomes, reliable tools for identifying patients at an increased risk of POAF are lacking. LASr has been associated with a higher risk of atrial fibrillation in other clinical settings; however, its role in predicting POAF is less well established. We aimed to assess whether LASr is associated with POAF risk after cardiac surgery.
MethodsThe literature was searched in PubMed, Embase, and Cochrane up to December 2024. The primary outcome was the predictive value of LASr for POAF in cardiac surgery. Secondary outcomes included the same analysis stratified by the type of surgery.
ResultsTwenty studies were included in the review, 18 of which, encompassing 1675 patients, were eligible for the meta-analysis. The mean age was 64.4 years in the POAF group and 60.5 in the NO-POAF group, and male sex was 70.7% and 69.5% respectively. LASr was significantly lower in the POAF group across all studies (21.4% vs 29.1%). Higher LASr was significantly associated with a lower risk of POAF (OR, 0.82; 95%CI, 0.77–0.89), and also in the coronary artery bypass surgery (OR, 0.84; 95%CI, 0.76–0.91) and valvular surgery (OR, 0.64; 95%CI, 0.48–0.87; HR, 0.96; 95%CI, 0.94–0.98). Heterogeneity was high and the certainty of the results was graded as low.
ConclusionsA higher LASr may carry a decreased risk of POAF in adult patients undergoing cardiac surgery. (PROSPERO ID number CRD42024627341).
Explorar la asociación entre la deformación del reservorio auricular (LASr) y la fibrilación auricular posoperatoria (FAPO) en los pacientes adultos sometidos a cirugía cardiaca. La FAPO es una complicación frecuente de la cirugía cardiaca en los adultos. Si bien se asocia a resultados clínicos adversos, no se cuentan con herramientas lo suficientemente confiables para identificar a los pacientes con mayor riesgo de desarrollar esta arritmia. La LASr se ha asociado a mayor riesgo de fibrilación auricular en otros contextos clínicos; sin embargo, su rol como predictor de FAPO no es tan claro. El objetivo fue estudiar si la LASr se asocia al riesgo de FAPO.
MétodosSe realizó una búsqueda de la literatura en PubMed, Embase y Cochrane hasta diciembre de 2024. El resultado primario fue el valor predictor de LASr para FAPO en cirugía cardiaca. Los resultados secundarios incluyeron el mismo análisis, pero estratificado según el tipo de cirugía.
ResultadosSe incluyeron 20 estudios en la revisión, de los cuales 18, con un total de 1.675 pacientes, fueron elegibles para el metanálisis. La edad media fue de 64,4 años en el grupo FAPO y 60,5 en el grupo NO-FAPO, y el sexo masculino fue del 70,7 y 69,5%, respectivamente. La LASr fue menor en el grupo NO-FAPO en todos los estudios (21,4 frente al 29,1%). Una mayor LASr se asoció significativamente a menor riesgo de FAPO (OR=0,82; IC95%: 0,77-0,89), así como en los estudios de cirugía de revascularización miocárdica (OR=0,84; IC95%: 0,76-0,91) y cirugía valvular (OR=0,64; IC95%: 0,48-0,87; HR=0,96; IC95%: 0,94-0,98). La heterogeneidad fue alta, y la certeza de los resultados fue calificada como baja.
