Introducción y objetivos

El flujo de la orejuela izquierda (FOI) refleja de forma indirecta la función auricular y puede servir como marcador de auriculopatía. Nuestro objetivo fue analizar la asociación del FOI con la aparición de eventos arrítmicos en pacientes con fibrilación auricular (FA) tratados con ablación con catéter (AC).

Métodos

Estudio observacional en el que se incluyó a pacientes consecutivos con historia de FA sometidos a AC. La determinación del FOI se realizó con ecocardiografía transesofágica previa a la AC. El objetivo principal fue el combinado de arritmias auriculares (FA, flutter o taquicardia auricular) de duración >30segundos durante el seguimiento, excluyendo un periodo de cegamiento de 2meses.

Resultados

Se incluyó a 198 pacientes (66,5±9,4 años; 38,9% mujeres). EL FOI promedio fue de 46,2±19,0cm/s, el tercer tercil (T3) se correspondió con 34,8cm/s. El FOI presentó un área bajo la curva de 0,72 (0,65-0,80) para la aparición del objetivo principal, el punto de corte óptimo fue 45,1cm/s. La presencia de un FOI <T3 se asoció con la aparición del evento principal (HR: 4,65; IC95%, 2,24-9,67; p<0,001) durante un seguimiento medio de 33meses. En el análisis multivariado, el FOI se asoció con la aparición de recurrencias arrítmicas tanto como variable continua (HR ajustada: 0,97; IC95%, 0,96-0,99; p=0,024) como dicotómica (HR ajustada: 3,03; IC95%, 1,32-6,94; p=0,009). Esta asociación se mantuvo consistente en el análisis de subgrupos.

Conclusiones

Un FOI reducido se asoció con la aparición de recurrencias arrítmicas en pacientes con FA tratados con AC.

Introduction and objectives

Left atrial appendage flow (LAAF) reflects atrial function and may serve as a marker of atrial cardiomyopathy. In this study we aimed to evaluate the association between LAAF and arrhythmic outcomes in patients with atrial fibrillation (AF) undergoing catheter ablation (CA).

Methods

We conducted an observational study including consecutive patients undergoing first-time CA for AF. LAAFV was measured via transesophageal echocardiography prior to ablation. The primary endpoint was the composite of atrial arrhythmias (AF, flutter or atrial tachycardia) >30seconds after a 2-month blanking period.

Results

A total of 198 patients (66.5±9.4 years; 38.9% females) were included. Mean LAAFV was 46.2±19.0cm/s and third tertile (T3), used to define low LAAFV, was 34.8cm/s. LAAFV presented an area under the curve of 0.72 (0.65-0.80); P<.001 and 45.1cm/s was the optimal cutoff. Low LAAFV was associated with the primary outcome (HR, 4.65; 95%CI: 2.24-9.67; P<.001) during a median follow up of 33months. In a multivariable model, both LAAFV as continuous variable and dichotomized were independently associated with AF recurrence (adjusted HR, 0.97; 95%CI, 0.96-0.99; P=.024 and 3.03; 95%CI: 1.32-6.94; P=.009, respectively). The association remained significant across subgroups and rhythm status during echocardiography.

Conclusions

Low LAAFV was associated with arrhythmic recurrence in AF patients undergoing CA.
